For individual agents:

• If already licensed with other insurance companies, submit the following:
  
  
  
  1. Duly filled-up Insurance Commission application for license (Handwritten)
  2. Copy of latest Income Tax Return (ITR)
  3. Copy of latest Professional Tax Receipt (PTR)
  4. License fee of P 500.00
  5. Four (4) 1x1 ID pictures
  6. Filled up Commission Crediting Form (available from BPI/MS Agency Management Division)
  7. If with trade name but solely owned by an individual, a copy of their DTI certificate of registration


• If not yet licensed in any non-life insurance companies
  
  
  1. Attend the Basic Non-Life Seminar conducted by BPI/MS, Insurance Institute for Asia and the Pacific (IIAP) or any other non-life insurance company
  2. Take and pass the licensure exam conducted by Insurance Commission or IIAP
  3. Comply with the requirements in nos. 1-7 in the first bullet

For corporate agents:

• If with license from other insurance companies:
  
  
  
  1. All board members and the Soliciting Official (SO) should be cleared from any Negative Database (NDB) findings
  2. Submit duly filled up Certificate of Authority (CA) application form, to be signed by the SO
  3. Submit a copy of the SO's ITR , PTR and four (4) 1x1 ID pictures
  4. Copy of the corporation's Securities and Exchange Commission (SEC) registration certificate, Articles of Incorporation and By-Laws, Audited Financial Statement, VAT registration certificate and latest ITR.
  5. CA fee of P 500.00
  6. Board resolution appointing the SO of the corporate agent


• If a newly formed corporation, without license from other insurance companies:
  
  
  1. Requirement from numbers 1-6 above
  2. The appointed SO should be a holder of a valid license as an individual agent prior to his appointment as SO. If not, the SO should go through the licensing procedure of an individual agent and wait for a year before applying as a corporate agent.
